A new vaccine that protects against all forms of TB in infants, children and adults, including HIV-infected persons, is desperately needed. More than a dozen candidate vaccines are in clinical trials, but development is hampered by imperfect animal models and lack of an immunologic correlate of vaccine-induced protection. Experimental medicine studies in humans to test unconventional concepts, e.g., protection of infection, may hold the key to advancement into large-scale efficacy trials.
